In order to remain financially viable, providers must ensure that reimbursement is timely, accurate, and properly reflects the services rendered to patients.  Proper coding and documentation is absolutely essential to ensure that the severity of the patient's illness is truly reflected in the medical record, and translated properly to the UB-92.  Payers will continue to scrutinize claims for accuracy, and reject those with discrepancies.  Providers whose coding practices are not precise may be leaving reimbursement dollars on the table.
